| ANNUAL MEMBERSHIP FEE | Due at time of registration. This fee ($35.00 - per person, $55.00 per family) is not included in the class fee.

| CLASS FEE | The semester tuition Fee may be paid in full upon enrollment or in 3 monthly installments (authorized credit card withdrawal or a bank draft only, service fee per transaction applies). Auto draft can not be used after first week of class. The semester fee will be pro-rated for students beginning after the first week of the semester.

| REFUNDS | Are given when a student withdraws within first week of the session.

| PAYMENTS | We offer a variety of payment options: MASTERCARD, VISA, bank draft, check, or cash. Make checks payable to FIRST STATE GYMNASTICS INC and write the name of person attending class, day and time of the class on each check. A service charge of $30.00 will be assessed for each RETURNED CHECK! A monthly FINANCE CHARGE of $25 will be added to the balance owed if payment is not received 7 days after balance is due. If a schedule conflict arises, we will try our best to transfer your child to a different class.

| DISCOUNTS | We offer a 15% discount for additional family members, with no charge for the fourth child. In addition, any child attending a second class per week receives a 25% discount on the second class. Annual Registration fee is not discounted.
